What is a hemoperfusion?

The development of modern hemoperfusion therapies began in the 1980s. Extensive data and experience reports are available on their effectiveness. Whole blood procedures (more blood-friendly), volume-neutral and higher flow rates enable a safe and effective procedure. The duration of treatment is 4 hours on average. During this time, depending on the flow rate and the weight of the patient, 72-120 L of blood can be purified (approx. 16-27x total blood volume). ). Depending on diagnosis and indication: 2-4 treatments. No relevant removal of immunoglobulins and lipids.

Haemoperfusion is an extracorporeal blood purification process that uses highly biocompatible plastics that are able to bind different substances due to their physicochemical properties. This process is called adsorption or hemoadsorption. We use adsorbers that have a specific or non-specific, i.e. broad adsorption spectrum.

The adsorber with the broader adsorption spectrum works in a concentration-dependent manner. This means that at high concentrations, larger quantities of cytokines, chemokines, toxins and decomposition products etc. can be removed very quickly. The higher flow rates support the effectiveness of this treatment.

The elimination rate decreases with decreasing concentration. This concentration-dependent autoregulation is intended to counteract the complete removal of physiological substances.

In contrast to plasmapheresis, blood components and plasma are not separated by a plasma separator during hemoperfusion. This is why this treatment is also referred to as a whole blood procedure and volume-neutral. A distinction is made between specific and non-specific adsorption and cytapheresis.

Hemoperfusion is an extracorporeal, minimally invasive procedure. These therapies are effective but also strenuous, which is why an accompanying person should always be present.

As anesthesia is not required for hemoperfusion, you do not need to be fasting. Eat as usual and make sure you drink enough fluids.

As this is a minimally invasive procedure, puncturing with cannulas is required for hemoperfusion. This puncture causes minimal pain in the short term, which can be minimized with local anaesthetic and/or intravenous pain therapy. Hemoperfusion procedures are painless.

On the day of the hemoperfusion and the following 3 days, physical exertion should be avoided as far as possible and sufficient fluid intake should be ensured.

The duration of treatment is normally 4 hours.

The day after the hemoperfusion, we perform a wound check of the puncture site of the Sheldon catheter. No further treatment is normally required after this minimally invasive procedure.

Depending on the diagnosis, indication and procedure, between 1 and 5 hemoperfusions are required.
